Surgical Management in Malignant Polyp of Colon


Abstract
The surgical management of malignant polyp was the controversial issue according to varying criteria. Some authors advocated resection whereaso thers did not. Also some suthors suggested that endoscopic polypectomy had favorable prognosis. Many
attempts
have been made to clarify the management principles. A clinical review was carried out 9 cases malignant polyp patients who received operation in the department of surgery at the Ewha womans University Hospital from October 1989 to February 1991.
@ES The results were as follows;
@EN 1) Mean age was 45.3(range 24 to 67). Men were four and women five.
2) The chief complains were rectal bleeding 6, melena 2 and abdominal pain one.
3) All patients underwent colonoscopy and two pateints were familial polyposis.
4) Distribution was as follows; rectum 4, sigmoid 2, transverse colon 1 and rectosimoid 2(familial polyposis).
5) The mean maximum diameter was 17 mm(range 5 to 20 mm)
6) Pedunculated polyps were 6 and sessile 1. Two familial polyposis had both types.
7) Four patients demonstrated to tubular adenoma, three patients demonstrated to villotubular adenot a and two familial polyposis combined tubular and villoutubular adenoma.
8) Pathologic findings were as follows; two patients were carcinoma in situ, other seven patients were invasion of muscularis mucosa. Lymphatic vessels and vascular invasion were not identified histologically. Differenciation was well or
moderately.
9) Involvement of resection line of polyp was as follows; Only two patients were involved. Two were close to rescetion line and five were free.
10) Management were as follows; endoscopic polypectomy 2, explorative colotomy and polypectomy 1, segmental resection 2, A-P resection 2, Total proctocolectomy and ileostomy 2(familial polyposis)
11) Postoperative complication was one case and it was intestinal obstruction.
12) Mortality was not in all cases.
